چکیده
Histopathological studies on the cerebral aneurysm in cerebrovascular moyamoya disease have been limited to only a few cases and the pathological details still remain obscure. Six moyamoya disease autopsy cases that satisfied the criteria of this disease proposed by Nishimoto in 1976 are studied. In addition to the routine neuropathological examination of the brains, detailed observa tion of the circle of Willis and its branches was carried out under a dissecting microscope and all of the bulged areas or locally thin portions of the vessel wall were histologically examined. All the pa tients had cerebral aneurysms. Seventeen aneurysms in total were discovered and their diameter ranged from 450 to 1, 300 ,um (mean diameter of 860 μm) . The principal histology of these aneurysms was of the saccular type in which the dome was composed of endothelial and adventitial layers, accompanied by the disappearance of the internal elastic lamina and the media. These small saccular aneurysms of the cerebral arteries in moyamoya disease were revealed to have the following characteristic features: 1) They were located at the bifurcating sites of arteries, and were distributed predominantly on the posterior communicating and posterior cerebral arteries, and on the abnor mal vascular networks branching from these arteries. 2) They were usually small, about 1 mm in diameter in size, and were multiple. 3) The lumen of the aneurysms was often occluded by fibrous thickening of the intima, which is believed to be the most characteristic disease process in moyamoya disease, and is commonly noticed in various cerebral arteries, most significantly at the terminal portion of bilateral internal carotid arteries. Based on these results, the pathogenesis of these small saccular aneurysms in moyamoya disease, and a possible mechanism in cerebral aneurysm formation were discussed.

Magnification Angiography of Cerebral Aneurysms Associated with Moyamoya Disease
The true cause of intracranial hemorrhage in moyamoya disease has not been clarified. Magnification cerebral angiograms were reviewed for seven patients. In four , small saccular aneurysms were seen arising from abnormally dilated collateral vessels or feeding arteries. This study suggests that aneurysms may be a source of hemorrhage in addition to the generally accepted rupture from abnormally...
متن کاملIntracranial saccular aneurysm and moyamoya disease.
Moyamoya disease is a rare but well described entity which has been found in the angiographic investigation of subarachnoid hemorrhage, its most common symptom in adults. We present 4 patients in whom moyamoya disease and an intracranial saccular aneurysm were discovered. In 2 of the 3 patients suffering a hemorrhage, the aneurysm was the source of bleeding. Three of the aneurysms were located ...
متن کاملExacerbation of Major Artery Aneurysms after Revascularization in Patients with Moyamoya Disease: Two Case Reports
The natural history of aneurysms of the major arteries after revascularization in moyamoya disease has yet to be documented. At our institute, we treated two patients with moyamoya disease-associated aneurysms involving major arteries of the posterior cerebral circulation. The aneurysms became enlarged at an early stage after revascularization, necessitating coil embolization. Although cerebral...
متن کاملEvolution into moyamoya disease in an infant with internal carotid artery aneurysms
Introduction Moyamoya disease (MMD) is characterized by progressive stenosis and occlusion in the terminal portion of both internal carotid arteries (ICAs) and the formation of an abnormal vascular network. Because of the fragile structure of the collateral vessels, MMD is frequently accompanied by intracranial aneurysms that are mainly located within the abnormal basal network or the circle of...
متن کاملTreatment Strategies for Management of MCA-M1 Spontaneous Dissecting Aneurysms
The majority of MCA aneurysms are saccular lesions that arise at the MCA bifurcation region [1-5]. Aneurysms involving the M1 segment are less common, accounting for approximately12% of MCA aneurysms, and are generally small saccular lesions at the origin of the anterior temporal artery or a lenticulostriatal perforator [6,7]. MCA-M1 fusiform lesions vary significantly from their saccular count...
